What Are Intrusive Thoughts?
Intrusive thoughts are unwanted thoughts, images, or urges that pop into your mind without warning. They can be disturbing, feel out of character, or cause intense anxiety. Common themes include:
- Fear of harming someone (even when you never would)
- Religious or blasphemous thoughts
- Sexual thoughts that feel “wrong” or unsettling
- Obsessions with contamination, symmetry, or morality

Intrusive Thoughts and OCD
Many people don’t realize that intrusive thoughts are often a symptom of Obsessive-Compulsive Disorder (OCD) or an anxiety disorder. OCD is not just about organizing things or washing hands—it may show up in the form of “Pure O” (Pure Obsessional OCD). In this form of OCD, the compulsions may be mental rather than physical, and they’re harder to spot from the outside.
People with Pure O may engage in:
- Mental rituals like reviewing memories, praying repetitively, or trying to “neutralize” a thought
- Avoidance of situations, people, or places that trigger intrusive thoughts
- Constant self-reassurance or Googling to determine whether they are “a bad person”
Even without visible compulsions, the inner turmoil is real—and exhausting.
